
You qualify to become a FirstCare Advantage (HMO) member if:

You are entitled to Medicare part A and continue to pay your Part B premium if not otherwise paid by a third party enrolled in Part B (medical coverage)

Crosby, Floyd, Garza, Hale, Hockley, Lamb, Lubbock, Lynn, or Terry

Jones or Taylor

You do not have End Stage Renal Disease (certain exceptions apply).

FirstCare Advantage (HMO) is a Health Plan with a Medicare Contract which renews annually. Availability of coverage beyond the end of the year is not guaranteed. If Medicare determines during the year that our contract must be terminated, then the plan's members will be notified in advance of the plans termination. You must be entitled to Medicare Part A, be enrolled in Part B, and reside in our service area to be eligible. Enrollment is limited to certain times of the year. Please see eligibility restrictions for more guidance.
If FirstCare Advantage (HMO) terminates the contract with CMS, a notice to all members will be provided within 60 days prior to the termination. If CMS elects to non-renew the FirstCare Advantage (HMO) contract, a notice to all members will be provided within 30 days prior to the termination. The notice will include a list of alternatives available in the area for obtaining Medicare services. In addition, FirstCare Advantage (HMO) will publish a CMS approved notice of the contract termination/non-renewal in the local newspapers.
